Long-Term Outcomes of Endoscopic Resection versus Open Surgery for Locally Advanced Sinonasal Malignancies in Combination with Radiotherapy

Abstract Objective Our objective was to compare the long-term outcomes of endoscopic resection versus open surgery in combination with radiotherapy for locally advanced sinonasal malignancies (SNMs). Methods Data continuous patients squamous cell carcinoma and adenocarcinoma who received (endoscopic or surgery) combined our center between January 1999 December 2016 were retrospectively reviewed. A 1:1 matching propensity scores performed. Overall survival (OS), progression-free (PFS), local recurrence rate (LRR) evaluated. Results We identified 267 eligible patients, 90 whom included after matching: 45 endoscopy group group. The median follow-up time 87 months. In group, 84.4% intensity-modulated (IMRT), a mean gross tumor volume (GTV) dose 68.28 Gy; 64.4% IMRT, GTV 64 Gy. 5-year OS, PFS, LRR 69.9, 58.6, 24.5% 64.6, 54.4, 31.8% respectively. Multivariable regression analysis revealed that surgical approach not associated lower LRR. overall postoperative complications 13% while 21.7% Conclusion For SNMs, minimally invasive resection, higher radiation new techniques such as yields similar those radiotherapy.
